| For Lost / Stolen Cards |

| 1. | Report your lost/stolen BPI Express Cash Card to 89-100 immediately. |

| 2. | Claim reimbursements for unused value after 90 days via issuance of Manager's Check or issuance of a new card. |

| For Expired Cards |
| |  |
| 1. | Renew your card by calling 89-100 or by visiting the branch where you purchased your card.
(a fee of P100 will be charged every month after the 121st day of card's expiry) |

| For Damaged Cards |
| |  |
| 1. | Report and surrender your damaged BPI Express Cash Card to 89-100 or at the branch where you purchased your card. |

| 2. | Replacement card* can be claimed 5 days from BPI's receipt of request. (*BPI will replace your card for FREE, provided that the card has still its full original value and has never been used. Otherwise, BPI shall issue a replacement card with value equal to the remaining balance of damaged card less the replacement fee.) |
